You will be required to fill out COVID-19 pre-screening questionnaire prior to each visit. If the questionnaire is not complete, I will ask you to remain outside until it is completed. If you answer “yes” to any of the questions please stay home and self isolate until you can prove a negative test or asymptomatic for min 14 days. Your appointment can be reschedules and no fees will be charged.
